Research into who actually uses the product and what they are trying to finish, information architecture, wireframes, high-fidelity designs, a reusable component system, and a handover that developers can build from without guessing. For existing products we also run usability review against the live screens, which usually finds more value faster than a redesign.

Yes, and it is a large share of what we do. We work in place rather than proposing a rewrite: audit the current screens, find where people get stuck or give up, then re-skin and restructure incrementally. Staff keep working throughout instead of being handed an unfamiliar system overnight.

Yes. Colour contrast to WCAG AA, real labels on every form control, keyboard reachability, visible focus states and markup that produces a sane accessibility tree for screen readers. For government and institutional work this is frequently a procurement requirement, not a nicety — and it is cheaper designed in than retrofitted.
